Candlewood Valley Health and Rehabilitation is looking for a full-time social worker to join our team!
We are located 18 miles from Danbury!
Candlewood Valley is currently looking for qualified Social Worker to join our team. Looking for caring and compassionate candidates to provide our residents and their families with the superior care delivered by our staff dedicated to the principles of kindness, compassion, customer service and excellence. Candlewood Valley is a 5 Star facility. Located in New Milford CT. We have 148 beds, with two long-term units, a secured memory care unit and a short-term unit. Qualifications:
- Bachelor's degree from a four-year college or university and/or training. Master's Degree preferred.
- Possess general office and computer skills
- Must be proficient in Matrix charting
- Ability to work independently and to instruct and motivate others.
- Must be proficient in ASCEND
- Must have experience in discharge planning
- Must have knowledge of Medicare, Medicaid, and private insurances
- Possess initiative, attention to detail and excellent follow through.
Responsibilities:
- Work with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing and/or aiding in the access of services to meet those needs.
- Complete social service assessment, care plan, and discharge plan for residents.
- Coordinate necessary services for a resident's discharge.
- Participate in in-service and continuing education programs.
- Counsel newly admitted residents and their families; discuss problems, needs and goals.
- Regularly visits with residents and assess their needs.
- Responsible for fostering a climate, policies and routines that enable residents to maximize their individuality, independence and dignity.
- Provide residents with the highest practical level or physical, mental and psychosocial well-being and quality of life.
