Job Summary
The QA of Medical Records is responsible for monitoring all medical records processes according to the current workflow to provide organizational storage for patient documents. The QA must ensure that all patient charts are complete, up to date, free of duplicates, and errors. They must ensure that the medical records coordinators assigned have received, sorted, identified, saved, and uploaded faxed records into the EHR; including requested labs, diagnostics, specialist’s notes and prior medical history from providers and facilities. Must assess the medical records coordinator’s technical accuracy and customer service performance. This individual will assist in monitoring, auditing, and implementing the medical records department’s quality processes and procedures. Will work closely with the team leads and management team to discuss audit reviews and provide recommendations of any training enhancements as needed.
Essential Duties & Responsibilities
- Review and audit the quality of work performed by assigned medical records coordinators through their daily productivity and monitoring the use of call recording system and applications.
- Monitoring the receiving, requesting, uploading, attaching and linking of all health-related documents including medical notes, test results, labs, state forms, patient letters, insurance referrals, insurance letters, pain assessment, hospital records, previous medical history and all other documents related to the patient’s health into the EHR software.
- Ensure that all necessary information is uploaded to the patient’s chart within a timely manner.
- Maintain patient confidence by keeping patient records information confidential.
- Develop quality strategies to help the organization achieve strategic goals.
- Provides actionable data to team leads and the management team for internal support.
- Prepares and analyzes quality reports for auditing the medical records department staff.
- Pursue continuing education on new solutions, technology, and skills.
- Perform other related duties as assigned.

- Document and keep record of audited charts.
- Ensure each chart is accurate, completed, and current.
- Provide reporting on a timely basis.
- Disclose results to management team.
- Revise policies and procedures for changes with the Medical Records Department.
Education & Experience
- Bachelor’s degree preferred.
- Minimum of 1 year of Medical Records Services or Quality Assurance experience required.
- Minimum of 1 year of training experience preferred.
Knowledge, Skills & Proficiencies
- Skilled in operating phones, computers, software such as (ECW, Five9), along with Microsoft Office platforms (Word, Excel, Power Point).
- Strong organization, training, and process management skills.
- Strong collaboration and relationship building skills.
- Excellent written and verbal communication skills.
- Ability to learn new tasks and concepts.
- Requires the ability to work independently and establish priorities.
- Fluent in English and Spanish (preferred).
Physical Requirements
This position works under usual office conditions. The employee is required to work at a personal computer as well as be on the phone for extended periods of time. Must be able to stand, sit, walk, and occasionally climb. The incumbent must be able to work extended and flexible hours and weekends as needed. Physical demands include ability to lift to 50 lbs. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
