Community Habilitation Specialist
Rate of Pay : $17.00 - $18.70 (depending on experience)
Full Time, Part Time and Per diem positions available, located Chautauqua County - Jamestown area.
SUMMARY OF DUTIES
As a Community Habilitation Specialist, you will work directly with individuals with developmental / learning disabilities, providing services to the people we support so they can live independently and thrive in their communities. This could include, but is not limited to :
- Community integration
- Community travel and transportation
- Money management and budgeting
- Appropriate socialization / interactions
- Healthy lifestyle skills / exercise
- Medical appointments and understanding doctor's orders
- Additional skills needed to enable the individual to access the community and remain as independent as possible
- Other instruction, services and supports as indicated by your supervisor.
SKILLS & ABILITIES
EDUCATION & TRAINING
Preferred : One year pertinent experience in health and human service related field and experience with individuals with disabilities
Minimum : High school diploma or equivalent
Valid New York State Driver's License in good standing and use of personal vehicle for transporting individuals receiving services.
