Job Description:
We are seeking an experienced Admission Registered Nurse (RN) to join our team. The successful candidate will be responsible for administering nursing care for patients of all ages in their place of residence, coordinating care with the interdisciplinary group, patient, family, and referring agency, and assuming responsibility for coordination of care.
Required Skills and Qualifications:
To be eligible for this role, you must be a Registered Nurse licensed per state requirements, with at least 2 years of experience with a Hospice agency preferred. Additionally, you should possess good communication skills, assessment skills, strong clinical documentation skills, ability to read, write, and speak English, and reliable transportation with valid and current auto liability insurance.
Key Responsibilities:
The successful candidate will be responsible for:
1. Conducting initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient's physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs.
2. Applying specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient's eligibility.
3. Implementing/developing/documenting the plan of care to ensure quality and continuity of care and recommending revisions to the plan as necessary.
4. Consulting with and educating the patient and family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs. Prioritizing any needs with the members of the IDG. Providing training to other staff as needed.
