Job Summary
">As a Hospice Care Professional, you will play a vital role in delivering compassionate care to patients with terminal illnesses. You will work collaboratively with interdisciplinary teams to coordinate patient care and ensure the highest quality of life for our patients.
">Responsibilities
">- Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs
- Develop and implement individualized plans of care to meet patients' unique needs
- Consult with and educate patients and their families on disease processes, self-care techniques, and end-of-life care
- Prioritize patients' needs and develop effective care plans in collaboration with the interprofessional team
- Provide training and education to other staff members as needed
Qualifications
">To be successful in this role, you will need:
">- Current RN licensure in good standing
- Minimum 2 years of experience in hospice care or a related field
- Strong communication and assessment skills, as well as excellent clinical documentation abilities
- Ability to read, write, and speak English fluently
- Reliable transportation and a valid driver's license
