Job Title: Registered Nurse (RN) – Long-Term Care 6:30pm-7:00am
Job Summary
The Registered Nurse (RN) will provide high-quality skilled nursing care to residents, following the medical directives of the attending physician and ensuring compliance with state nursing practice standards. This role involves patient assessments, physician communication and order follow through, overseeing State Tested Nursing Assistants (STNAs) and licensed nurses, and ensuring the highest level of care.
Key Responsibilities:
Administrative Duties
- Oversee STNAs and licensed nurses as directed by the Director of Nursing.
- Collaborate with the interdisciplinary team to create and update individualized care plans.
- Ensure completion of baseline and periodic assessments.
- Assist with physician rounds, including chart preparation and physician order documentation and completion.
- Submit consultation and referral requests as needed.
- Perform administrative tasks, including filling out medical forms, reports, and evaluations.
Clinical Duties
- Provide direct nursing care following the facility’s policies and professional care standards.
- Monitor resident health for acute changes in condition and perform appropriate assessments.
- Assist with resident admissions, orientations, and discharge planning.
- Administer medications per practitioner orders and report any adverse reactions.
- Monitor and document resident weight, food, and fluid intake; report significant changes.
- Coordinate with hospice for end-of-life care and promote fall prevention practices.
- Provide wound care, collect specimens, and adhere to infection prevention protocols.
- Ensure proper pain management and provide first aid when needed.
- Participate in resident assessments, documentation needed for care plans and MDS.
- Provide compassionate care to residents with cognitive decline and trauma history.
Committee Involvement
- Participate in quality assurance, infection control, safety, and risk management committees.
- Attend policy advisory and other facility committees as needed.
Requirements:
Education & Experience
- Graduate of an accredited nursing program (RN degree).
- Minimum of 2 years of experience as a Registered Nurse (RN) preferred.
Licensing & Certification
- Active, unencumbered RN license in the state.
- Current CPR certification.
- Must comply with all health and safety regulations.
Skills & Competencies
- Strong knowledge of long-term care regulations and nursing practices.
- Leadership and supervisory skills to guide STNAs and nursing staff.
- Ability to work well with a team and communicate effectively with healthcare professionals, residents, and families.
- Compassionate and patient, especially when dealing with elderly or emotionally distressed residents.
Working Conditions:
- Exposure to blood, body fluids, infectious diseases, and hazardous chemicals.
- Requires repetitive motion, prolonged sitting/standing, and ability to lift/move 25 pounds or more.
- Position consists of working every other weekend and holiday rotation.
