Compassionate LPN - Patient Care Advocate

Duties

• Provide direct patient care, including administering medications and treatments as prescribed by physicians.
• Monitor patients' vital signs and report any significant changes to the nursing staff or physicians.
• Maintain accurate patient records in compliance with HIPAA regulations using electronic health record systems.
• Collaborate with interdisciplinary teams in case management to develop and implement individualized care plans.
• Provide support for patients with dementia, ensuring their comfort and safety throughout their stay.
• Engage in patient education, teaching families about care plans and health management strategies.
• Work in an inpatient setting, contributing to a positive and supportive environment for both patients and staff.

Qualifications

• Valid Licensed Practical Nurse (LPN) license in the state of practice.
• Knowledge of physiology and nursing principles relevant to patient care.
• Strong communication skills, both verbal and written, with the ability to educate patients effectively.
• Familiarity with case management processes and documentation standards.
• Ability to work collaboratively within a multidisciplinary team while demonstrating empathy and professionalism.
• Previous experience in dementia care is an advantage but not required.

Pay: $23.00 - $30.00 per hour
