Job Summary:
We are seeking a dedicated and experienced Hospice RN Case Manager to join our team. The ideal candidate will be responsible for providing comprehensive nursing assessments, care planning, and coordination for patients receiving hospice services. You will collaborate with interdisciplinary team members to ensure the highest level of care for patients and their families.
Key Responsibilities:
- Manage a caseload of 12-14 patients, conducting thorough assessments and developing individualized care plans.
- Provide skilled nursing care according to established care plans and hospice regulations.
- Collaborate with physicians, social workers, and other team members to ensure holistic and coordinated care.
- Educate patients and families about end-of-life care options and available resources.
- Monitor patients’ conditions and adjust care plans as needed based on clinical assessments.
- Ensure compliance with all regulatory requirements and best practices in hospice care.
- Maintain accurate and timely documentation of patient interactions and care provided.
Qualifications:
- Current RN License in Colorado.
- Minimum of 3 years of nursing experience, with preferred experience in hospice or palliative care.
- Strong assessment, communication, and organizational skills.
- Compassionate and patient-centered approach to care.
- Ability to work independently and manage time effectively.
