Responsibilities of Nurse Case Manager
- Evaluate patients (existing, new and prospective) based on their needs and limitations
- Address patient concerns and goals while maintaining consistent communication with the patient and their nursing team
- Build rapport with patients and their families
- Coordinate care with PT and nursing departments
- Develop care plans for patients

Requirements for Nurse Case Manager
- Covid vaccination
- Registered Nurse or Licensed Practical Nurse licensure in Massachusetts
- Experience as a Case Manager in a health care setting preferred.
- Possesses and demonstrates competence in an identified area of clinical expertise.
- Excellent verbal and written communication skills, expressing self in a clear, concise, and professional manner.
- Ability to work collaboratively and effectively with interdisciplinary team members, patients, families, and external case managers.
- Excellent time management and organizational skills.
- Analytical and strong problem- solving skills.
- Ability to work effectively and efficiently under tight deadlines, high volumes and multiple interruptions.
