Salary : $4,791.00 MonthlyLocation : Wilmington - Downtown Campus, NCJob Type : Full Time StaffJob Number : 23-00075Division : Economic & Workforce DevelopmentOpening Date : 02 / 10 / 2025Closing Date : 2 / 26 / 2025 11 : 59 PM Eastern Position DescriptionDEFINITION : The PH2 Career Case Manager will perform a variety of complex technical and administrative duties including coordinating and facilitating employment and education related services to support individuals returning from incarceration, including employer engagement, data collection, enrollment processes, job placements, review of records, and advising to potential students for the Pathway Home 2 grant.

  • SUPERVISION RECEIVED AND EXERCISED : Receive general supervision from the Pathway Home 2 Program Director and direction from the LINC Lead Case Manager. Duties & Responsibilities (Essential Functions)ESSENTIAL AND OTHER IMPORTANT FUNCTIONS STATEMENTS : Essential and other important responsibilities and duties may include, but are not limited to, the following : Essential Functions : Perform regular visits with incarcerated and formally incarcerated individuals in collaboration with PH2 Lead Case Manager to begin development of Individual Career Plan.Request information about clients from appropriate sources.Complete and document applications / referrals forms for client services.Review client files, notes and documentation prior to appointments to familiarize self with prior employment, education, and achievements or progress.Recruit new employers to the PH2 grant providing with resources and incentives to offer second chance opportunities.Engage with current PH2 employers regularly identifying skills required for current positions.Refer participants to appropriate jobs by connecting them with current and new employers known to support second chance hiring.Prepare and present on behalf of CFCC. Ensure timely, accurate, sufficient data is entered into the grant tracking system.Provide strength-based customer service in a manner grounded in empowerment, encouragement and motivation.Present self as open, friendly, inviting, engaging and enthusiasticPractice reflective listening skills, listening in a manner to identify needs and re-phrasing information heard for clarification.Manage time as appropriate for providing intensive case management for a caseload in accordance with Case Management policies and procedures.Maintain system for enrolling / scheduling and tracking clients in education and employability skills development training, including but not limited to : mock interviews, conflict resolution, communication skills, maintaining employment, job advancement, presentation skills, and social skills.Inform clients about their status and your efforts on his / her behalf.Assist the Director in planning outreach events in the community.Update client records to reflect changes in status.Seek and develop relationships with existing and potential partnering service providers.Interact and communicate effectively with various groups and individuals including immediate supervisor, co-workers, subordinates, and the general public.Communicate on an on-going basis with the Lead Case Manager regarding general questions, concerns, feedback, and regarding any client issues, concerns, and questions.Follow policies and procedures; input various information into reentry software; download, print, sort and file materials as appropriate.Participate in and contribute to employee performance evaluations and staff development opportunities as scheduled by the Pathway Home 2 Program Director.Participate in and contribute to scheduled staff meetings.Communicate, in accordance with confidentiality laws and regulations, with service providers about the client and significant others in his / her life Other Important Functions : Be informed of current best practices for working with reentry population.Remain familiar with and implement evidence-based assessment tools.Follow-up with employers regarding hired participants to provide strategic support to overcome obstacles and / or identify opportunities for career advancement.Disseminate information, provide outreach, and training to potential employers.Network with businesses, agencies and organizations to remove employment barriers and resolve issues related to community integration and the independence of people returning from incarceration.Identify, attend and introduce clients to job fairs, workshops, conference, training and online resources for employment opportunities.Other duties, as assigned QualificationsQUALIFICATIONS : Knowledge of : Extensive knowledge in case management.Basic principles of report preparation and data entry in to reentry platform.Knowledge of group facilitation, behavior dynamics, reentry & incarceration Settings, and substance abuse recovery.Knowledge of current treatment modalities and community resources. Modern office procedures, methods and computer equipment.Advanced word processing methods, techniques and programs MS Office and Google Suite.Advanced spreadsheet and database knowledge MS Office Access, Excel, and Google SheetsAbility to : Interpret and apply the policies and procedures of the function to which assigned.Maintain compliance records for college. Perform accounting activities including the maintenance of appropriate files and compiling information for reports.Operate a personal computer, calculator and other office equipment. Type and word process at a speed necessary for successful job performance.Respond to inquiries and requests for information.Maintain effective audio-visual discrimination and perception needed for : Making observationsReading and writingCommunicating with othersOperating assigned equipmentHandling varied tasks simultaneouslyMaintain physical condition appropriate to the performance of assigned duties and responsibilities, which may include the following : Standing or sitting for extended periods of timeOperating assigned equipmentHandling varied tasks simultaneouslyBending, stooping, climbing, or lifting up to 50 poundsMaintain mental capacity, which allow the capability of : Making sound decisionsResponding to questionsDemonstrating intellectual capabilitiesEXPERIENCE AND TRAINING GUIDELINES : Any combination of education, work experience and life experience preferred. Requiresextensive knowledge in case management, group facilitation, behavior dynamics, reentry & incarcerationsettings, substance abuse recovery and knowledge of current treatment modalities and community resources. A typical way to obtain the knowledge and abilities would be : Minimum Experience : Associate's Degree in human services related fieldFive years of case management experienceExperience working with justice involved individualsMicrosoft Office experience (including Word, Excel, and PPT) Google Suite experience (including Docs, Sheets, Drive)Preferred Education / Training, Experience, Skills : Bachelor's degree in social work or human services related fieldColleague experience preferredHandshake experience preferred Supplemental InformationSpecial Instructions to Applicants : This position REQUIRES UNOFFICIAL TRANSCRIPTS AND / OR TRAINING CERTIFICATIONS be submitted in addition to the application.
