Works within the Cumberland County EMS Community Paramedic Program to identify patients with substance abuse and mental health issues during times of crisis from a social work perspective and provide appropriate intervention, recommendations and support to patients and their families as well as referral to other agencies as required. The Social Worker will work alongside other members and contribute to the multi-disciplinary team to ensure that the best possible treatment and service delivery is offered and provide follow-up and support to ensure successful outcomes for patients. The SW II is a fully integrated team member who provides highly individualized services in the community and promotes individual self-determination and decision-making. The SW II also provides essential expertise and consultation to the Community Paramedic team, peer agencies, families and patient to promote a culture in which each individuals point of view and preferences are recognized, understood, respected and integrated into treatment, rehabilitation, and community self-help activities.$5,000 Sign on bonus
