GENERAL SUMMARYProvides administrative support to program case management.ESSENTIAL JOB FUNCTIONS

• Provides home visit relief to Nurses and Case Managers when needed.Holds a small case load, and performances Case Manager role for that case load.Audits charts for completion.Completes and updates identification sheets and photos.Follows up via telephone to request missing caregiver logs / financial transaction records (FTR) and other essential case file information.Tracks six month summaries and physician’s orders.Tracks fire drills, home inspections, furnace inspections and safety plans.Tracks caregiver trainings.Files paperwork.Tracks Individual Service Plans (ISP) and participant census via caregiver logs.Tracks selected individuals’ financial information (RFMS).Serves as key contact for Adult Foster Care determination information.Completes paperwork for new program participants / caregivers.Ensures adequate supply of commonly used forms.Manages incoming and outgoing mail.Maintains daily staff schedule.
