Key Responsibilities:
The Resource Nurse at Capital Caring Health plays a crucial role in supporting the Clinical Manager in the day-to-day operations of the nursing team. Key responsibilities include:
- Triage referrals and admissions, ensuring efficient and effective patient assignment
- Participate in IDT meetings, contributing to the development and implementation of patient plans of care
- Maintain accurate and comprehensive medical records, adhering to regulatory and accreditation standards
- Collaborate with multidisciplinary teams to enhance patient care and service delivery
Requirements:
- Minimum three years of Hospice/Home Health experience, with management experience and medical surgical and/or oncology experience preferred
- LPN education from an accredited nursing program, with BSN RN preferred
- Licensure as a nurse with District of Columbia, State of Maryland, and/or Commonwealth of Virginia licensure required
- Driver's license in good standing and access to a vehicle during working hours
