Job Description:
We are seeking a skilled and compassionate Resource Nurse to join our team at Capital Caring Health. As a key member of our nursing staff, you will play a critical role in ensuring the delivery of high-quality patient care.
Responsibilities:
- Assist the Clinical Manager in triaging referrals and admissions, assigning patients to appropriate team members, and monitoring patient care
- Participate in IDT meetings, reviewing and completing meeting notes, and assisting with medical record audits to ensure quality care and compliance
- Liaise between Hospice and other professionals, including AMDs, community physicians, hospitals, nursing homes, and agencies
- Provide education and support to team members, enhancing their clinical practice and integrity
- Develop new skills through ongoing education and maintain knowledge of current nursing practice
Qualifications:
- Minimum three years of Hospice/Home Health experience, with at least two years of management experience and four years of medical surgical and/or oncology experience preferred
- LPN education from an accredited nursing program, with BSN RN preferred
- Licensure as a nurse with District of Columbia, State of Maryland, and/or Commonwealth of Virginia licensure required
- Driver's license in good standing and access to a vehicle during working hours
