About the Role:
The Resource Nurse is a vital part of our nursing team at Capital Caring Health. You will work closely with the Clinical Manager to ensure the smooth operation of daily activities, including referral triage, admissions, and patient assignments.
Main Responsibilities:
- Coordinate patient care and services, ensuring timely and effective delivery of care
- Collaborate with multidisciplinary teams to develop and implement patient plans of care
- Monitor patient progress, identifying areas for improvement and making recommendations for enhancements
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
Requirements:
- Three years of Hospice/Home Health experience, with management experience and medical surgical and/or oncology experience preferred
- LPN education from an accredited nursing program, with BSN RN preferred
- Licensure as a nurse with District of Columbia, State of Maryland, and/or Commonwealth of Virginia licensure required
- Driver's license in good standing and access to a vehicle during working hours
