The CDPHP pharmacy residency program provides a unique opportunity to engage in the advancement of the Triple Aim (patient experience, effectiveness and efficiency) with CDPHP. The resident will be involved with formulary and policy development, Medication Therapy Management and population health management across all lines of business, pharmaco-economics/management and clinical pharmacy services for medical home/enhanced primary care sites. The CDPHP pharmacy residency program is a one year post-graduate training experience and is consistent with the requirements set forth by ASHP and AMCP. Upon completion of the program, the resident will have an extensive understanding of managed care pharmacy with enhanced clinical skills. At the completion of the residency program, the pharmacist will be a well versed participant of the health care team, in support of primary care, and able to identify the high value activities that a pharmacist can uniquely contribute and that will deliver measurable value in quality and efficiency.  

Qualifications:

Annual salary 56,000.00

CDPHP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. In addition to cash compensation, CDPHP employees may be eligible for an incentive payment, a discretionary cash reward based on employee and company performance. Some roles may also be eligible for overtime pay.
