Role:The Per Diem Dietitian is responsible for the assessment and implementation of a plan for dietary counseling of identified hospice patients as requested by clinical staff. Plans food and nutrition program for Hospice Houses.
Qualifications:
• Bachelor’s degree in dietetics, foods and nutrition, or food service systems management or an equivalent combination of education and experience
• Licensed in the State of Florida as a Licensed Dietitian
• Registered Dietitian from the Academy of Nutrition and Dietetics (formerly American Dietetic Association)
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy
• Ability to travel to off-site locations
• Professional attitude with good leadership skills
• Ability to work unsupervised in the field, exercising a high degree of discretion and independent judgment
• Well developed interpersonal skills: ability to problem solve with staff, while maintaining excellent working relationships
• Able to communicate clearly in English

Job Responsibilities:
• Assesses, plans, documents and provides dietary counseling for individual patients as requested by clinical staff.
• Implements nutritional care plans based upon hospice philosophy and patient/family treatment goals.
• Plans nutritionally balanced and appropriate menus for patients in the Hospice Houses, considering factors such as palatability, attractiveness, availability of food supplies, and budget implications.
• Provides staff education on regulatory requirements of the dietary needs of patients, when requested. Provides other specific staff education, when requested.
• Consults with physicians, managers and patient care nurses in order to coordinate specific patient’s medical and nutritional needs.
• Consults with Hospice House managers regarding applicable inpatient unit sanitary and safety regulations.
• Prepares and maintains required documentation for each assigned patient. Completes documentation in a timely manner and distributes to the appropriate parties.
• Performs other duties as assigned.
