Responsibilities:
- Assist in evaluating clients’ needs and developing appropriate service plans
- Support clients with accessing community resources (housing, food, transportation, employment, etc.)
- Document client interactions and maintain accurate case records
- Help coordinate appointments, follow-ups, and referrals
- Participate in home visits or facility visits as needed
- Collaborate with social workers, healthcare professionals, and other service providers
- Perform administrative tasks, including filing, data entry, and compiling reports
- Help advocate for clients’ rights and ensure services are delivered equitably
- Maintain client confidentiality in compliance with HIPAA and agency policies
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Sociology, or related field
- Prior experience in social services or human services is preferred
- Familiarity with California social service systems is a plus
