Summary:
We are looking for exceptional individuals who are motivated and committed to providing outstanding service each and every day to children and families in Mercer County. Key responsibilities include:
Job Responsibilities:
- Facilitate Child and Family Teams
- Provide face-to-face contact with clients at or above minimum quantity standards
- Provide telephone contacts with clients at or above minimum quantity standards
- Provide crisis on-call coverage as needed (CCCC provides 24/7 on-call crisis services for all CCCC clients)
- Provide crisis intervention services to children and families as needed. Provide initial crisis intervention and management with linkage to other professional services as needed
- Coordinate and facilitate comprehensive assessments of the strengths and needs of children and families in their community
- Facilitate the development of ISP’s (Individual Service Plans) for children and their families that allow and respect individual choice, and is outcome-based
- Coordinate Care Management across multi-systems
- On an ongoing basis, assess the quality of all services being offered to clients
- Maintain accurate, timely, and comprehensive records for each child and family
- Develop transition plans for the child and their family into other services
- Other duties as assigned by supervisor or higher-level management personnel
- Qualifications
- Bachelor's degree (Required); Master's degree (a plus)
- One or more years of experience in a related position, preferably with at-risk children & families. (Required)
- Prior experience in managing wraparound/individual services. (Required)
- Developmental Disabilities (DD) or Intellectual Developmental Disabilities (IDD) experience. (Strongly Preferred)
- Substance Abuse experience. (Would be a plus)
- Valid driver's license. (Required)
- Excellent communication (verbal and written), interpersonal, organizational, and assessment skills. (Required)
- Possess energy, creativity, and cultural competence. (Required)
- Company car provided for driving to families' homes. (Must have a good driving record)
- Able to work under supervision and as part of a team. (Required)
- Bilingual language skills (Spanish or other languages (s)) in addition to English strongly (preferred).
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
