JOB DESCRIPTION SUMMARY

The Clinical Documentation Specialist is responsible for maintaining support of clinical documentation including management of accuracy, improvement, and completeness of documentation.

QUALIFICATIONS

  • Physical Therapist Assistant (PTA) with current and unencumbered license to practice in the state of employment.
  • Associates' degree in Physical Therapist Assistant (PTA) from an accredited program, required.
  • Minimum of three (3) to five (5) years' experience in clinical documentation review; experience in home care operations, preferred.
  • Prior experience using Microsoft Office Suite and electronic medical documentation software.
  • Proficient in medical terminology.
  • Superior time management and organizational skills.
  • Ability to communicate effectively and efficiently with clinicians and administrative staff.
  • Access to reliable transportation.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  • Demonstrates knowledge of clinical documentation practices.
  • Improves the overall quality of clinical documentation and thoroughness to ensure that appropriate reimbursement is received for the correct level of service rendered to all patients.
  • Supports the accuracy and completeness of clinical information used for measuring and reporting outcomes.
  • Follows all safety rules. Reports all accidents and safety hazards identified in clinical documentation within the established time-frames to appropriate managers.
  • Conducts follow up reviews of clinical documentation to ensure points of clarification have been recorded in the patient record.
  • Identifies opportunities for performance improvement based on reliable data trending; recognizing and analyzing undesirable patterns or trends in performance.
  • Participates in performance improvement activities as requested. Effectively utilizes software applications.
  • Accountable for accurate and timely review of documentation and meeting caseload requirements.
  • Assists with special projects as needed.
