JOB DESCRIPTION SUMMARY
The Clinical Documentation Specialist is responsible for maintaining support of clinical documentation including management of accuracy, improvement, and completeness of documentation.

QUALIFICATIONS
- Physical Therapist Assistant (PTA) with current and unencumbered license to practice in the state of employment.
- Associates' degree in Physical Therapist Assistant (PTA) from an accredited program, required.
- Minimum of three (3) to five (5) years' experience in clinical documentation review; experience in home care operations, preferred.
- Prior experience using Microsoft Office Suite and electronic medical documentation software.
- Proficient in medical terminology.
- Superior time management and organizational skills.
- Ability to communicate effectively and efficiently with clinicians and administrative staff.
- Access to reliable transportation.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
- Demonstrates knowledge of clinical documentation practices.
- Improves the overall quality of clinical documentation and thoroughness to ensure that appropriate reimbursement is received for the correct level of service rendered to all patients.
- Supports the accuracy and completeness of clinical information used for measuring and reporting outcomes.
- Follows all safety rules. Reports all accidents and safety hazards identified in clinical documentation within the established time-frames to appropriate managers.
- Conducts follow up reviews of clinical documentation to ensure points of clarification have been recorded in the patient record.
- Identifies opportunities for performance improvement based on reliable data trending; recognizing and analyzing undesirable patterns or trends in performance.
- Participates in performance improvement activities as requested. Effectively utilizes software applications.
- Accountable for accurate and timely review of documentation and meeting caseload requirements.
- Assists with special projects as needed.
