The Outreach & Enrollment Coordinator is responsible for facilitating access to quality health care and social services, providing health education, conveying linguistically and culturally responsive health care directly to communities, assisting patients and/or individuals to become equal partners in their health care, and increasing community awareness of health center programs, services, and the needs of the underserved population to improve quality of life.
Job Duties
· Responsible for overall increased patient visits.
· Perform Enabling Services such as Case Management. Credentialed by supervisor or his/her designee as being knowledgeable of the functions and demonstrates competency in performing these duties.
· Facilitate access to quality health care and social services. Coordinate patient appointments. Refer individuals and/or patients to resources such as housing assistance, food assistance, utility assistance, employment assistance, transportation assistance, etc.
· Convey information in a manner that is linguistically and culturally responsive to patients, communities, and individua being served.
· Provide information, application and enrollment assistance, and assistance regarding health plan coverage or determination to individuals seeking coverage through Medicaid, All Kids/CHIP, Health Insurance Marketplace, etc.
· Provide referrals to state and or federal agencies such as Medicaid, All Kids/CHIP, etc. as needed.
· Complete Certified Application Counselor (CAC) training program and pass exam.
· Complete Medicaid Assister training program and pass exam.
· Coordinate with Outreach Enrollment Manager for developing and maintaining community relationships such as local organizations, schools, community service agencies, churches, and businesses to increase awareness of health center programs and services.
· Coordinate with Outreach Enrollment Manager for speaking engagements, marketing and public relations activities, educational opportunities, workshops, events, and other functions to target and educate individuals of health center programs and services.
· Report outreach and enrollment efforts in Electronic Medical Record including community referrals, enabling services, enrollment assistance, patient education, outreach activities, and promotional tools.
· Provide services in the strictest of confidence and abide by the Health Insurance Portability and Accountability Act (HIPAA).
· Comply with all state and federal privacy, security, and conflict of interest standards.
· Perform other duties as assigned.
Job Requirements
Education
· High School diploma or equivalent. Must be able to speak read, write, and understand English.
Professional
· Demonstrated success in communication, customer service or working with the general public, preferably in a medical care facility. Must be LGBTQ Sensitive.
· Demonstrated success in managing difficult customer situations.
· Demonstrated success in general computer competence including basic Word and potential to be trained on specific software for patient information, billing, and communication.
Language Ability
· Ability to read and interpret common reports and documents.
· Ability to respond to common inquiries or complaints.
· Ability to write clinical notes and summaries in prescribed style and format.
· Ability to communicate in both Spanish and English is preferred.
