Position: Care Team Coordinator
As the Care Team Coordinator, you will play a pivotal role in organizing and overseeing the care teams that support our clients. Your efforts will ensure that exceptional care is provided, helping clients maintain their independence and safety in their homes. You will work closely with the management team to ensure that services are delivered efficiently and with the highest standards of care.
Key Responsibilities:
- Staff Coordination: Oversee and manage the scheduling of care team members, ensuring appropriate coverage for all client needs.
- Team Support: Provide ongoing support to the care team and management to ensure seamless operations and delivery of care.
- Facility Operations: Open and close the facility, providing assistance with daily office operations as needed.
- Administrative Duties: Assist with office functions such as document filing, maintaining office supplies, and performing general administrative tasks in compliance with company policies and regulatory standards.
- Client Care: Ensure that care teams are well-equipped and supported in their efforts to assist clients in maintaining a safe and independent living environment at home.
- Additional Duties: Perform any other responsibilities as directed by management to ensure smooth daily operations and high-quality care.
Requirements:
- Valid driver’s license.
- High School Diploma or GED (required).
- Ability to travel up to 15% of the time.
- Strong organizational and communication skills.
- Ability to work collaboratively in a dynamic team environment.
