Job Summary
We are seeking a dynamic Primary Care Medical Director to lead our clinical strategy and operations. In this role, you will be responsible for designing and implementing innovative clinical workflows, developing population health initiatives, optimizing quality measure outcomes, and driving performance excellence across our primary care network.
You will balance direct patient care with strategic leadership, working closely with our multidisciplinary team of clinicians, designers, engineers, and operational experts to advance Carbon Health's primary care model. This position offers the opportunity to shape the future of healthcare delivery while maintaining connection to clinical practice.
Key Responsibilities
Clinical Workflow Design: Develop and implement efficient, technology-enabled clinical workflows that optimize both patient experience and provider productivity
Population Health Management: Create and lead population health initiatives that improve health outcomes and reduce costs across diverse patient populations
Quality Improvement: Establish quality metrics, monitor performance, and implement improvement strategies to ensure exceptional clinical outcomes
Performance Optimization: Drive clinical productivity and operational efficiency while maintaining high standards of patient care
Team Leadership: Build and mentor a high-performing team of primary care physicians and advanced practice clinicians
Care Pathway Development: Design evidence-based care pathways that deliver consistent, high-quality care across all service channels
Clinical Practice: Provide direct patient care (approximately 1 shift/week) to maintain clinical skills and stay connected to frontline healthcare delivery
Qualifications and Skills
Unrestricted licensed physician, BC/BE MD/DO in Family Medicine or Internal Medicine in the state of California or Massachusetts
8 years post-residency direct patient care and clinical experience in primary care
3 years of experience as a Medical Director of primary care or medical group
Demonstrated success in clinical workflow design and implementation
Proven track record in quality improvement and population health management
Experience with provider performance optimization
Strong data analytics skills and ability to translate insights into clinical strategies
Leadership experience managing clinical teams across multiple locations
