Program Description:
Cardea Health is an Oakland based nonprofit organization providing clinical staffing and program design that targets vulnerable populations in the San Francisco Bay Area, including people experiencing homelessness. Cardea Health operates clinical programs across a variety of sites including emergency shelter, medical respite, and permanent supportive housing.
Cardea Health is seeking a full-time Nursing Director of Transitional Housing Programs to support and supervise nursing staff across transitional housing programs including medical respite, short term post hospitalization shelter, and behavioral health housing. The Nursing Director will work closely with the Chief of Clinical Operations to ensure the delivery of quality consistent care that improves the health and quality of life of our clients.
Qualifications
Required Qualifications:

Hold an associate or bachelor’s degree in nursing

Hold current license as Registered Nurse through the California Board of Registered Nursing

Current Basic Life Support Certification
-
Current two years in a managerial or supervisory nursing position
-
Experience with homeless or incarcerated populations is highly desirable
Essential duties and responsibilities:
-
Ensure that clinical care delivered by the Cardea nursing team reflects the mission and values of Cardea Health
-
Develop and enact quality improvement and quality assurance programs appropriate for clinical programs
-
Communicate clinical policy initiatives and changes to clinical team across programs
-
Partner with Cardea Health clinical staff and community providers to develop individualized plans of care. Arrange and attend case conferences with collaborating partners for complex clients

Work with team members to develop creative solutions to meet the needs of individual clients
-
Oversee the collection and maintenance of data on program operations for reporting and analytical purposes. Participate in the analysis and reporting of program data
-
Ensure adherence to infection control standards on site among staff and residents
-
Participate in population health interventions among clients that contribute to safety and health including measures designed to stop the spread of Covid-19
-
Standardize clinical documentation practices according to site policies and scope of care
-
Identify needs for nursing education and training, develop ongoing education programs
-
Supervise the orientation and onboarding process of new nursing staff in collaboration with nursing supervisors
-
Working in collaboration with the Cardea Health medical billing team, oversee the submission of documentation required for reimbursement and billing procedures
-
With the support of the Cardea Health human resources department, participate in workforce recruitment, performance evaluation, and performance improvement interventions as needed
-
Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA)

Recognize and respond to medical emergencies
-
Practice harm reduction principles, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect

Salary : $150,000 - $165,000
