Cardea Health is a nonprofit healthcare organization founded to connect marginalized populations to the clinical and supportive services they need to improve their health, remain stably housed in their community, and age in place. Cardea Health’s programs fill a critical – and growing – gap in the system of care. Among people experiencing homelessness, a growing number are living with complex medical needs. They face poor quality of life, increased morbidity and mortality, and medical and social destabilization. But they have few housing options: they need a higher level of care than what is offered through permanent supportive housing, but they do not wish to live in an institutional setting and have limited access to hospice or palliative care.
Cardea Health specifically targets this vulnerable population, providing a range of clinical services within permanent and transitional housing sites to both address their medical needs and allow them to live independently in their communities, addressing a critical need within the system of care.
Job Summary:
The Housing Program Manager supervises and manages the housing team, supports resource linkage, and develops housing plans for individuals who have experienced chronic homelessness. The Housing Program Manager helps individuals navigate the complex housing system, identify the appropriate resources they will qualify for, and help them successfully exit the medically supported shelter program in a timely manner. The Housing Program Manager coordinator will serve as a leader in Cardea Health’s housing work and works closely with operational leaders across programs to build and improve housing processes. The housing program manager will oversee staff use of HMIS for data entry and reporting.
Cardea Health staff collaborate with other housing programs and social services to create a safe, independent living environment for a dynamic and vulnerable population. The ideal candidate will be invested in Cardea Health’s mission of serving marginalized populations with compassion and empathy. The housing program manager will need to be flexible, patient, and willing to work in challenging environments with a diverse population. Experience with homeless or incarcerated populations is highly desirable. This is an excellent position for someone who is seeking to gain experience in a healthcare social service setting.
Primary Location:
Eddie's Place located in the Hegenberger neighborhood of Oakland, near the Oakland Airport and Coliseum. It is a mixed use site providing pos post-hospitalization medically supported shelter and medical respite services. Eddie's Place provides skilled nursing care, caregiving services and connections to behavioral health care, including substance use services.
Qualifications
Minimum Qualifications:
- Bachelor’s degree or equivalent work experience
- Self-directed and able to work with minimal supervision.
- Detailed oriented and highly organized.
- Must provide proof of COVID vaccination or willing to be vaccinated.
- Prior experience with homeless or incarcerated populations
- Proficiency in Microsoft Outlook, Word and Excel is required
- Must have experience with housing placement through the coordinated entry system, and other subsidized housing resources; preferably in alameda County.
Essential Duties and Responsibilities. Includes both supervisory responsibilities and direct provision of housing services, depending on program needs
Supervisory
- Manages the housing team.
- Creates and manages housing workflows and processes.
- Manages caseload assignments for the housing team.
- Supports team in identifying appropriate discharge plans/locations for all clients.
- Manages throughput and ensures clients are being discharged within their anticipated length of stay.
- Manages collaboration between the housing and clinical teams.
- Works closely with the Eddie’s Place Program Director to determine strategy for the housing work at Cardea.
- Facilitate HMIS training for staff, and ensure that staff are appropriately entering required data
Direct client services
- Manages a caseload of shelter residents (this number will vary depending on the needs of the site).
- Completes screenings and assessments to identify housing needs and factors contributing to housing instability, including functional, environmental, employment, educational, and health information, as appropriate.
- Develops housing plans for each resident and ensures they are on track to successfully exit the program by or on their anticipated discharge date.
- Assists shelter residents on caseload to become document ready for future permanent housing options if appropriate.
- Helps connect residents to Housing Resource Centers (HRCs) and complete Coordinated Entry System (CES) assessments.
- Facilitates shelter referrals.
- Completes and maintains documentation of their work using the standard work developed by Cardea Health.
- Develops and maintains collaborative relationships with providers, facilities, community resources, etc.
- Serves as a strong advocate to help shelter residents obtain the housing resources they are eligible for.
- Maintains confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
- Practices harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect.
- Other duties and responsibilities as required.
