What are the responsibilities and job description for the Student Program Coordinator position at Cardinal Cushing Centers?
Student Program Coordinator will coordinate the delivery of special education services and will be the primary contact for the parent/guardian, school based team members, funding agencies and outside providers. The Student Program Coordinator will work with a student’s team to ensure that special education and related services are provided to the student as outlined in their IEP and to assist each student achieve the goals listed on their IEP.

PRIMARY DUTIES AND RESPONSIBILITIES
•Responsible for overall communication with parent/guardian, school district and outside agencies. This includes an established communication plan (i.e.,notebook, email, phone log, etc.)
•Facilitate internal meetings including: internal team collaboration, IEP, progress, transitional, service planning, etc.
•Day to day coordination across environments on an as needed basis to allow students to successfully access their educational program
•Available, flexible, responsive in order to support the student, staff, and families needs
•Responsible for ESPED management of IEP’s and ensure that parent and student concerns and vision is reflected in the document
•Oversee development of the IEP process to ensure Cardinal Cushing Centers meets all mandatory timelines in order to remain in compliance with MA state regulations and DESE compliance
•Ensures that information regarding student’s safety and welfare are communicated to parents and all team members (i.e. behavioral incidents, medical incidents, programmatic changes)
•Communicate regularly with all staff in order to ensure that IEP goals and objectives are being implemented and student needs are being met across all settings
•Leads Service Planning Meetings for incoming students and work with admissions staff to assist with transition of new students into the program
•Uses clinical/educational skills to problem solve, create and implement interventions to improve student outcomes
•Will serve as a resource to families through transition process (i.e. 688 process, guardianship, Rogers, etc.) in order to access appropriate services within the necessary time frame.
•Collaborate with outside agencies/families in all aspects of the transition process to other programs or reintegration to program (i.e. return from inpatient hospitalization, move to less restrictive environment, internal program transfer, transfer to adult services etc.)
•Ensure parents/guardians and school districts receive incident reports after communicating with them by phone and/or email
•Gather information from the team members upon report of allegations of abuse or neglect. If necessary, files official report with appropriate protective agency (i.e. DCF, DPPC, etc.) and informs parents directly of filing. Maintains all documentation relevant to the incident. Ensure the student safety remains paramount throughout the process.
•Intervenes physically in behaviorally aggressive situations using Safety Care

•Regular attendance and promptly reporting to work at the beginning of the scheduled shift are essential functions of this position
•Assumes additional responsibilities as assigned
Salary : $50,000 - $65,000
