Job Summary
We are seeking a compassionate and dedicated Hospice Nurse to join our exciting new team. The ideal candidate will provide high-quality patient care to individuals with life-limiting illnesses, ensuring comfort and dignity during their end-of-life journey. This role requires a strong understanding of patient care standards, effective communication skills, and the ability to work collaboratively within a multidisciplinary team.
Duties
- Deliver comprehensive nursing care to hospice patients in accordance with established protocols and standards.
- Assess patient needs, develop individualized care plans, and implement appropriate interventions.
- Monitor vital signs and manage symptoms related to infectious diseases and other health conditions.
- Provide education and support to patients and their families regarding disease processes, treatment options, and end-of-life care.
- Collaborate with physicians, social workers, and other healthcare professionals to ensure holistic patient care.
- Maintain accurate documentation in electronic health records systems such as Athenahealth.
- Assist with durable medical equipment (DME) setup and management as needed.
- Lift and transfer patients safely while adhering to best practices for patient mobility.
- Participate in ongoing training and professional development to stay current with hospice care standards.
Experience
- Current nursing license in the state of practice.
- Previous experience in hospice or palliative care preferred but not required.
- Familiarity with coding systems related to patient care documentation is a plus.
- Strong understanding of patient care standards and best practices in nursing.
- Ability to ventilate patients when necessary, demonstrating competence in critical situations.
- Excellent interpersonal skills for effective communication with patients, families, and team members.
