Job Description

Licensed Practical Nurse (LPN)

Location : Cass and Fulton County

Pay : $30.30 - $33.33 per hour

Summary :

As an LPN at Cardinal Services, you will provide a wide range of nursing tasks under the direction of an RN. You will play a crucial role in ensuring the health and well-being of clients, using your judgment to implement care plans and solve problems. The LPN will work with moderate supervision and collaborate with agency staff, service providers, and the public.

Duties and Responsibilities :

Compliance & Standards :

Adhere to regulatory standards such as Indiana Nurse Practice Act, Medicaid Waiver, and ISDH.
Ensure compliance with all applicable policies and procedures.
Provide nursing care in line with the agency’s standards, and complete required data reviews.

Client Care & Nursing :

Manage a caseload consisting of Medicaid Waiver clients and those in group homes.

Conduct routine monitoring, face-to-face visits, and consultations with clients.

Develop and adjust risk management plans and train staff on their implementation.

Maintain medical / nursing charts, documenting interventions and contacts.

Perform emergency care, when necessary, until emergency responders arrive.

Training & Education :

Provide in-service training to staff, clients, and families on health-related topics.

Assist with medical appointments, gather required documentation, and manage referrals.

Team Collaboration & Health Coordination :

Coordinate with healthcare professionals (doctors, pharmacists, dietitians, etc.) and interdisciplinary teams for optimal client care.

Communicate with care teams regarding medications and updates.

Assist with client transitions and discharges.

Representation & Communication :

Represent Cardinal Services positively, ensuring effective communication with participants, families, and the public.

Maintain regular communication with health professionals for coordinated care.

Qualifications :

Licensure : Current Indiana LPN licensure required.

Experience : At least 1 year in ID / DD or long-term care nursing preferred.

Skills : Ability to effectively communicate, follow written and oral instructions, and assist clients with their needs.

Certifications : CPR / First Aid, Medication Administration, Positive Behavioral Support Training (required or willing to obtain).

Physical Requirements : Ability to perform moderate to heavy lifting, and support individuals with medical and behavioral challenges.

Other : Valid driver’s license, insured vehicle, and willingness to travel within Cass and Fulton County and surrounding areas.

Additional Information :

Hours are Monday-Friday 7am-4 pm. Part of the on-call rotation with policy adherence.

Will be reimbursed for mileage when traveling within service areas.
