Transition Consultant
Location: Elkhart High School, Elkhart County Indiana
Pay: $21.63 an hour
Job Summary:
Cardinal Services is seeking a Transition Consultant for its Pre-Employment Transitions Services program. In this full-time role (40 hours/week), you will work with students with disabilities to facilitate their transition from high school to adult life. Your responsibilities will include developing plans for work-life skills training based on each student's Individualized Education Plan (IEP) and personal goals.
Key Responsibilities:
- Provide guidance and support to students aged 14 - 22 with disabilities.
- Develop personalized plans for skill-building and work experiences.
- Collaborate with students, families, and educational staff.
Job Requirements:
- Must be at least 18 years of age.
- High school diploma or GED equivalent required.
- Preferred: One year of experience working with individuals with intellectual/developmental disabilities (ID/DD).
- Preferred: One year of experience working with transitional youth.
