What you'll do
Primary objective is identifying, prioritizing and developing lead sources within assigned market; Management of referral sources in regards to leads, relationships and clinical assessments of referrals; Ensures smooth transfer/admission from the hospitals to CarDon Healthcare facilities for any appropriate patients from a primary referring hospital.
Pre-Admission Assessment and Service Coordination:
- Receive leads from providers, facilities, or any customer source, and converts appropriate leads to drive census.
- Manage and coordinate leads from key hospitals in the market.
- In conjunction with facility requirements/abilities, clinically assess patients to determine if the admission criteria is met.
- Make contact with potential leads as a representative of CarDon facilities.
- Review charts and assist with any questions from lead and/or family members.
- Obtain the necessary information (medical and financial) to qualify patients for admission.
- Troubleshoot areas of Admission; Public relations, pre-certification and physician follow up in conjunction with facility.
Sales and Marketing Coordination:
- Manage customer relationships in accordance with the facilities sales plan.
- Conduct regular visits to referring hospitals, build and maintain relationships with Discharge Planners, Case Managers, Physicians and all other potential referral sources.
- Initiate contact calls to identify new and potential business sources. Identify new physicians and assist with scheduling meetings with physicians.
- Maintains thorough knowledge of the market in general and the specific products, services and acuity trends within the market.
- Identify the options for partnering with other providers in the market and assist them to develop partnerships with necessary providers.
- Coordinates community education and events for hospitals, physicians and other referral sources.
- Follow up with referral sources with case studies and updates.
Our ideal candidate
- Must be a Registered Nurse or Licensed Practical Nurse, in good standing with the State of Indiana.
- Education in the healthcare field and previous experience in the long-term care field is desirable.
- Must reside in the Southern Indiana, Evansville market.
- Has knowledge of and special sensitivity to the needs of the aging.
- Projects a confident, creditable professional image.
- Able to communicate effectively with referral sources, clinicians, potential residents, and family members.
