Duties And Responsibilities
- Assists in oversight of Medicaid prospects
- Uploading obtained documentation within EMR
- Review documentation, notating any missing/incomplete items or potential service barriers
- Initiating contact with member or primary contact
- Notating details of need to include demographics, geographic location, language requirements, diagnoses, care and equipment needs, requested start, hours/utilization, or any other pertinent information that impacts care
- Being a subject matter expert on the Medicaid program and details of what is extended to our members
- Ensuring eligibility and providing resources if ineligible
- Obtaining necessary documentation by way of communication with member, primary contacts, Care Coordinators, Social Services, Facility Discharge Planners, etc.
- Point of contact for Managed Care Organizations, referral sources, and members/primary contacts as it relates to Medicaid Service Coordination
- Collaborating closely with the Client Services Department around the entering of prospective members
- Organization of where each prospect stands, notating barriers and being aware of how to overcome
- Weekly meetings to discuss all Medicaid prospects
- Encouraging interdepartmental collaboration to expeditiously onboard Medicaid members in need
- Generate weekly reports to ensure all departments are aware of where each prospect is in the process
- Communicate with branch teams and associated external resources through means of email, Teams messenger, fax, and telephone
- Maintain thorough progress documentation within EMR
- Manage department associated data and organization methods or reports
- Updating prospect status when appropriate to ensure accuracy of EMR
- Extending tasks to suitable parties
- Report out on progress when requested
