Role Description
This is a part-time hybrid role for an RN Care Manager, based in Knoxville, TN with some work-from-home flexibility. The RN Care Manager will be responsible for day-to-day tasks such as case management, coordinating elder care, conducting assessments, creating care plans, and providing support to clients and their families. Additional responsibilities include liaising with social service agencies, training CAB Care Partners, and ensuring the delivery of high-quality care.
Qualifications
- Case Management and Elder Care skills
- Experience in Social Services and coordinating with external agencies
- Medical knowledge and clinical expertise relevant to elder care
- Training skills for onboarding and educating Care Partners
- Strong interpersonal and communication skills
- Ability to work independently and with a team
- Current RN license in the state of Tennessee
- CCM preferred
- Experience in geriatrics is highly desirable
