As a Care Coordinator at Care At Home, LLC, you will play a vital role in ensuring our clients receive the highest quality of care. Responsibilities
- Managing scheduling tasks and addressing client scheduling needs to ensure scheduling goals are met and compliance with Care at Home LLC standards is maintained.
- Performing scheduling tasks, attending to client scheduling needs to ensure scheduling goals are being met and compliance is maintained to Care at Home LLC standards
- Staff client shifts and related issues, such as call-outs and restaffing, are promptly addressed.
- Coordinate care plans and services for clients based on individual needs and preferences.
- Act as a liaison between clients, families, and caregivers to ensure clear communication.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
- Conduct assessments to determine the appropriate level of care required for each client.
- Maintain accurate documentation of client information, services provided, and any changes in condition.
- Provide support to families by answering questions and addressing concerns regarding care services.
- Stay informed about industry best practices and regulatory requirements related to home care.
- Assist with orientation of new hires and incorporating them into the schedule.
Qualifications
- Strong understanding of medical terminology and healthcare practices relevant to home care.
- Experience in a high volume scheduling setting. (1 year required)
- Experience with scheduling in a home care environment. (1 year required)
- Excellent organizational skills with the ability to manage multiple tasks effectively.
- Strong communication skills for interacting with clients, families, and staff members.
