The Admissions Coordinator will report directly to the Administrator.
JOB DUTIES :
Assist in developing and implementing facility marketing plans Act as a liaison between the facility and hospital discharge planners Reach out to referral sources via phone Build relationships to identify unmet healthcare needs in the community and evaluate the facility's capability to meet those needs Complete all necessary paperwork for appropriate admissions Lead facility admission and census meetings Manage marketing material supplies and ensure information packets are readily available Prepare daily census reports Compile monthly and quarterly reports as directed Address inquiries from hospitals and the community Screen potential admissions : Determine payor source
Obtain necessary authorizations Verify Medicare eligibility Assess the level of care for appropriate placement Process admissions : Notify nursing staff and relevant personnel
Create identification bracelets for residents Provide charts to the appropriate nurse station Finalize admission contracts with residents or responsible parties Foster positive relationships with referral sources Maintain strong post-admit and ongoing relations with residents and their families Carry out additional tasks assigned by the Administrator Upload documents and ensure admission packets are signed by newly admitted residents Complete MC171, upload photos, gather all required information for admission, and upload PASRR File and label completed folders Cover breaks, lunches, and days off for Admissions and Reception as needed Update and organize the Covid-19 daily log to ensure completion Update and send Covid-19 notifications to Social Services.
