What are the responsibilities and job description for the LPN Nurse position at Care Centers of Nassau?
Position Summary:
Responsible for providing health care services to clients as identified by his/her health status, according to the Care Centers of Nassau’s policies and procedures, Florida Nurse Practice Act, and ICF/MR/DD rules and regulations. Assumes responsibility and accountability for assisting in the planning, organization, direction, supervision and evaluation of all nursing services in the facility.
Qualifications:
- Must hold a valid and current license to practice as a Licensed Practical Nurse or Registered Nurse in the state of Florida.
- Must hold a valid CPR, First Aid and AED card.
- Must be Insurable under agency vehicle insurance program.
- Must Pass Defensive Drivers Training Course
- Must be able to read and write in the English language.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Responsible to: This position works under the supervision and direction of the Director of Nursing (DON) and Supervising Nurse.
Essential Functions:The following are the essential functions of this position. Successful job applicants will be able to perform these essential functions with or without a requested reasonable accommodation:
- Check vital signs, review bowel histories, complete nursing summaries, perform Universal Precautions according to company policies and document information appropriately.
- Assessment of individual resident needs:
- Make rounds to assess each client at least every 2 hours while on duty.
- Report any emergency/urgent problem/issue to the Medical Director then notify the DON/Supervising Nurse.
- Always document client problems, action taken and outcome.
- Complete incident report and document all pertinent information in reference to the conditions of the incident, per facility Policy and Procedure.
- Prepare client for transport whether emergent or non-emergent and make sure all the required information and documentation is sent with the client, including needed supplies.
- Administer treatments and stay with the client while treatment is being administered (for example, 15-minute breathing treatment, the nurse must stay until the breathing treatment is completed).
- Management of client’s weight, ensure weekly and monthly weights are completed and accurately recorded, and reporting any significant weight loss/gain to Supervising Nurse or DON.
- Communicate with physician for all medical needs, implement physician’s orders as received.
- Educate staff regarding client and his/her diagnosis, treatments, mediations, etc.
- Comply with all policies regarding the reporting and prevention of abuse and neglect.
- Maintain client/resident confidentiality at all times.
- Prepare and administer medication, having full knowledge of the mediations being administered per current medication administration policy.
- Maintain and assume accountability for all schedule and controlled substances as defined by facility policy and pharmacy regulations:
- Record on MAR all medications, including PRN, and document controlled medications on the narcotic sheet.
- Always report any count discrepancies to the Supervising Nurse/DON.
- Assist in maintaining adequate meds/pharmaceutical stocks and supplies to provide continuity of administering meds and treatments:
- Order medications from pharmacy and communicate information to on-coming nurse.
- Initial and circle meds not given; record rationale and complete mediation error form.
- Report any missed dosages to nurse on call as soon as possible.
- Monitor expiration dates of mediations and place in appropriate area. If medications do not arrive, follow-up with pharmacy and notify Nursing Supervisor/DON.
- Review and make changes to the “Authorization for Refill Pharmacy Sheet” bi-weekly.
- Perform TB (PPD) skin tests, including completing all appropriate documentation.
- Check appointments, ensure packets are prepared for appointments and/or assemble packets for appointments. Transcribe any new orders when client returns from appointment.
- Participate in the communication process, verbal and written, of resident health and health care plan:
- Document nurse’s notes (computer entry), in a timely manner within the nursing guide lines of legal documentation.
- Give a full in-depth verbal report to on coming nurse. A written report is only acceptable in the event of an emergency situation.
- Maintain the security and cleanliness of the medication room and the nurse’s station:
- The nursing medication closet will remain locked at all times.
- Inventory and restock med rooms, organize med room in compliance with policies and procedures (internal meds separate from external meds, etc).
- Only authorized personnel are allowed in the nurse’s station and med room/closets.
- Each nurse is responsible for the nursing keys and should report immediately if any are lost/stolen.
- Keep med room and nurses station clean, odor free, empty trash every shift.
- Know, understand and comply with the set duties outlined in the Emergency Procedure.
- Inventory formula/supplies weekly and ensure all supplies when delivered are stored in a timely manner in each house.
- Attend and maintain all required training with due dates.
- Attend regularly scheduled nurse meeting
- Conduct duties in a professional manner to promote and support a team environment.
- Assist and participate in Interdisciplinary Meetings/HAB Plan Meetings as needed
- Follow the nursing department chain of command.
- Keep open, courteous, helpful and professional communication with other staff members.
- Any additional duties as assigned by DON/Supervising Nurse.
Physical Demands and Working Conditions:
- Must be able to work independently and have strong interpersonal skills.
- Sits, stands, bends, moves, stoops, and stirs intermittently during working hours.
- Must be able to lift, push, pull, and move equipment, supplies, etc., throughout the work day.
- Must be able to maintain professionalism with clients, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.
- Subject to hostile and emotionally upset clients, family members, etc.
- Speaking and hearing when receiving instructions and conversing with others
- Must possess sight/hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.
- Must be willing to work beyond scheduled hours, and in other positions temporarily, when necessary.
- Subject to odors throughout the work day.
- May be exposed to infectious waste, diseases, conditions, etc.
- Must be able to read and accurately record information.
