The LPN works under the guidance and oversight of the nurse case manager, to give direct nursing care to patients in the home within the policies of the Agency. The case manager is responsible for the LPN’s direction. The LPN provides nursing services, contributing to the treatment and rehabilitation of the patient. Consults with and refers families to appropriate personnel within the Agency or other community services as needed.
