Position Summary:
CareCollab is seeking a dynamic Housing Specialist to support Medicaid recepients in securing permanent and stable housing. The Housing Specialist will assess member eligibility, assist with completing necessary documentation, and aid in the development of skills for a successful housing transition. The role involves collaboration with Care Managers, the member’s Care Team and external agencies to ensure safe and affordable housing solutions.
Housing Specialist Responsibilities:
- Assist members with paperwork, accompanying them to interviews, and follow up with housing agencies to ensure the safety and affordability of housing units.
- Identify, outreach and establish agreements with housing providers and resources.
- Train Care Collab staff on housing issues, including 2010e application processes.
- Prepare housing-related reports and document services accurately.
- Assesses member needs and create service plans for appropriate housing, including supportive housing, independent housing, and various housing subsidies.
- Ensures timely preparation and delivery of housing packages and coordinateinterviews.
- Prepares clients for the housing interviews and assists with obtaining necessary entitlements and identification.
- Manage housing goals in the member’s Individualized Plan of Care.
- Meet documentation requirements using designated Care Management Portals andElectronic Health Records (EHRs).
- Build and enhance the housing network through community liaison work.
- Advocate for members within the agency and with external service providers.
- Communicate effectively with members, families, and caregivers, consideringlanguage, literacy, and cultural preferences.
- Participate in care planning meetings and interdisciplinary team members to effectively coordinate comprehensive and holistic care.
- Identify and manage community-based resources and referrals.
- Attend ongoing staff development training and required in-service seminars.
- Collaborate with external stakeholders (hospitals, shelters, jails, etc.) to achievepositive client outcomes.
- Performs additional duties as assigned.
Qualifications
- High School Diploma or Equivalent
- At least one year of housing or real estate experience with the NYC Department ofHomeless Services (DHS) or similar organizations, preferably with low-income,geriatric or homeless populations.
- Knowledge of city, state, and federal housing guidelines.
- Ability to research, network, and coordinate with community-based services.
- Experience working with homeless individuals, those with mental healthconditions, and substance abuse issues is preferred.
- Experience with NYC housing subsidies and programs is required
- Proficiency with computer operations (Microsoft Word, Excel, AWARDS, and Outlook).
- Excellent written and verbal communication skills.
- Bilingual abilities are a plus.
