Job Description

Job Title: Hospice and Palliative Care Director

Job Summary:

We are seeking a compassionate and experienced hospice and palliative care director to lead our team in providing quality end-of-life care to patients and their families. The director will oversee all aspects of patient care, develop and implement policies and procedures, and ensure compliance with regulatory requirements.

Responsibilities and Duties:

• Oversee the day-to-day operations of the hospice and palliative care program
• Develop and implement care plans in collaboration with physicians, nurses, and other healthcare professionals
• Provide leadership and mentorship to staff, ensuring high standards of care are maintained
• Monitor and evaluate the effectiveness of the program, making adjustments as necessary
• Collaborate with community resources and other healthcare providers to ensure comprehensive care for patients
• Serve as a liaison between patients, families, and the interdisciplinary care team
• Ensure compliance with state and federal regulations, as well as accreditation standards

Qualifications and Skills:

• Bachelor's degree in nursing, social work, or healthcare administration (Master's degree preferred)
• Certification in hospice and palliative care (CHPN, CHAP, or similar)
• Minimum of 5 years of experience in hospice or palliative care, with at least 2 years in a leadership role
• Strong communication, leadership, and organizational skills
• Knowledge of Medicare and Medicaid regulations
• Ability to work effectively in a fast-paced and emotionally challenging environment
