Job Title: LPN, Long Term Care (LTC)
Job Summary:
The LPN in Long Term Care (LTC) provides compassionate and individualized nursing care to residents in long-term care facilities. They work under the supervision of registered nurses and physicians to ensure the physical and emotional well-being of residents.
Responsibilities and Duties:
- Administer medications and treatments as prescribed by healthcare providers
- Monitor residents' vital signs and report any changes to the supervising nurse or physician
- Assist residents with activities of daily living, such as bathing, dressing, and eating
- Collaborate with the healthcare team to develop care plans and update resident records
- Provide emotional support and comfort to residents and their families
- Educate residents and families on health maintenance and disease prevention
Qualifications and Skills:
- Licensed Practical Nurse (LPN) certification
- Experience working in long-term care facilities preferred
- Strong communication and interpersonal skills
- Ability to work collaboratively with a multidisciplinary healthcare team
- Compassionate and patient-centered approach to care
