Job Summary:
We are seeking a highly skilled and compassionate Nurse Practitioner/Physician Assistant to join our Adult Medicine department. The ideal candidate will provide primary care services to adult patients, including diagnosing and treating acute and chronic illnesses, performing physical exams, and prescribing medication. This role requires strong clinical judgment, communication skills, and a commitment to delivering high-quality patient care.
Responsibilities and Duties:
1. Conduct comprehensive health assessments of adult patients.
2. Develop and implement individualized treatment plans based on patients' needs and healthcare goals.
3. Manage chronic conditions such as diabetes, hypertension, and heart disease.
4. Prescribe medications and order diagnostic tests as needed.
5. Collaborate with physicians and other healthcare providers to coordinate patient care.
6. Educate patients on preventive care and healthy lifestyle choices.
7. Document patient encounters accurately and in a timely manner.
8. Participate in quality improvement initiatives to enhance patient outcomes.
Qualifications and Skills:
1. Graduate of an accredited Nurse Practitioner or Physician Assistant program.
2. Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice.
3. Certification in Adult-Gerontology or Family Medicine preferred.
4. Minimum of 2 years of clinical experience in adult medicine.
5. Strong interpersonal and communication skills.
6. Ability to work collaboratively with a multidisciplinary team.
7. Proficiency in electronic health record systems.
