Job Summary:
We are seeking a dedicated and compassionate Social Worker to join our acute care team. The social worker will be responsible for providing support and assistance to patients and their families during their stay in the acute care setting. The ideal candidate will have a strong background in social work and a passion for helping others in times of need.
Responsibilities and Duties:
- Assess the psychosocial needs of patients and their families
- Provide counseling and emotional support to patients and families
- Coordinate discharge planning and aftercare services
- Collaborate with medical team to develop treatment plans
- Advocate for patients and assist with communication between patients and medical staff
- Provide resources and referrals for community support services
Qualifications and Skills:
- Master's degree in Social Work
- Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW)
- Experience working in an acute care setting preferred
- Strong communication and interpersonal skills
- Knowledge of crisis intervention and supportive counseling
- Ability to work effectively in a fast-paced and high-stress environment
