Job Summary:
We are seeking a compassionate and dedicated Social Worker MSW to join our hospice team. The role involves providing psychosocial support to patients and their families during the end-of-life process. The ideal candidate will have excellent communication and counseling skills, as well as a strong understanding of end-of-life care.
Responsibilities and Duties:
- Conduct psychosocial assessments of patients and their families to identify needs and develop care plans
- Provide counseling and support to patients and families in coping with terminal illness
- Coordinate with interdisciplinary team members to ensure comprehensive care for patients
- Advocate for patients' rights and assist with advance care planning
- Provide education and resources to help patients and families navigate the hospice process
- Maintain accurate and timely documentation of patient interactions and care plans
Qualifications and Skills:
- Master's degree in Social Work (MSW) from an accredited program
- Current licensure as a social worker in the state of practice
- Experience working in hospice or palliative care preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
