What are the responsibilities and job description for the LVN Field Nurse (Thermal) position at Care Dimensions of the Desert Home Health?
*Main office of Care Dimensions of the Desert is located in Palm Desert, but the coverage area for this position will be in Thermal, CA.*
Job Summary:
Utilizes the nursing process to perform skilled nursing functions in accordance with the
Nurse Practice Act, Federal and State guidelines. Provides direct skilled nursing care to patients in their home under the supervision of a Registered Nurse, supervised by the Director of Patient care Services and reports status. Provides services in accordance with Agency policies, standards and practices.
Responsible in following plan of care, provides treatment and works with the team to meet positive results for patient care.
Essential Functions:
1. Coordinates with home health services to the registered nurse/DPCS/Designee by informing the
needs for other disciplines - CHHA, MSW, PT, OT, ST, Registered Dietician.
2. Gives skilled nursing care in the home as ordered on the Medical Plan of Treatment and Nursing
Care Plan.
3. Performs aseptic techniques as required.
4. Performs specific treatments and administers medications as ordered.
5. Educates patients and significant others regarding the patient’s illness, procedures, treatments,
including self-care, prevention, and health maintenance.
6. Assists patient and/or family in keeping home and environment safe.
7. Keeps patients supplied with necessary materials and sees that charges are submitted for billing.
8. Prepares meals for patient appropriately.
9. Records information accurately, legibly, and concisely following the current Medical Plan of
Treatment.
10. Documents all changes in patient’s physical or psychosocial condition.
11. Attends staff meetings, patient conferences, and takes an active part in planning patient care as
needed.
12. Documents and notifies staff on changes in patient’s medications, treatments, physical and
psychosocial condition.
13. Provides discharge instructions to patient and family.
14. Keeps accurate visit records of time.
15. Attends and participates in continuing education programs for personal and professional growth
and development.
16. Adheres to all Company policies and procedures.
17. Demonstrates flexible and efficient time management techniques.
18. Prioritizes workload so as to meet productivity standards and applicable deadlines.
19. Maintains a positive and respectful attitude and demeanor.
20. Maintains strict HIPAA confidentiality with regard to protected and sensitive information.
21. Submits accurate and timely documentation in accordance to the plan of care.
22. Uses medical equipments and supplies efficiently and effectively.
23. Performs other duties or tasks as required as delegated by the RN Supervisor.
Knowledge, Skill and Experience:
1. Minimum Education required: Currently licensed by the California Board of Nursing to practice as a
Licensed Vocational Nurse.
2. Minimum Experience required: Minimum of one year in the acute care setting. Two years
experience in Home Health preferred.
3. With knowledge of compliance to accepted professional standards and practice.
4. Must be able to maintain strict HIPAA confidentiality with regard to protected and sensitive
information.
5. Must be able to solve problems independently and confidently by applying analytical and logical
thinking.
6. Skilled in assessing and reporting in an accurate, concise and professional manner.
7. With good oral and written communication skills.
