Summary
Reporting to the Palliative Care Clinical Manager, the Per Diem Pediatric Palliative Care RN is responsible for providing care coordination, care plan development, and anticipate and assist with symptom management including pain management for assigned pediatric palliative care patients. The Per Diem Pediatric Palliative Care RN participates in family support, teaching, discharge planning and continuity of care across sites of care, in collaboration with the primary care team.
What You'll Bring :
- Two year additional work experience as a professional pediatric nurse in a homecare, hospital or skilled nursing facility with strong palliative care assessment skills and care planning skills.
- One year work experience in hospice and / or palliative care.
- Must obtain and maintain any applicable certification for the performance of this functional role as defined by Care Dimensions.
- Demonstrated understanding of Medicare / Medicaid benefit as it relates to all facilities settings.
- Experience in the use of electronic medical record preferred.
- Strong verbal and written, communication skills with patients, caretakers and providers.
- Demonstrated ability to work independently with minimal supervision.
- Demonstrated Ability to work as a member of a multidisciplinary team.
- Demonstrated organizational skills, time management skills.
- Fluency in Spanish and / or Portuguese beneficial.

The Pay Range listed is the anticipated hiring range of this position. Precise pay rates are determined through consideration of one or more of the following : education, training, relevant experience and work location. There are some cases in which the pay rate could exceed the range listed on this job post. Pay Range
51-$54 USD
