Job Summary
We are seeking a dedicated and compassionate LVN for our Home Health division. Duties
- Conduct comprehensive assessments of patients' health status and develop individualized care plans.
- Administer medications and treatments as prescribed, including catheter care and cardiac catheterization when necessary.
- Monitor patients' vital signs and overall health, documenting changes and reporting to the healthcare team.
- Provide education and support to patients and their families regarding health management and disease prevention.
- Assist with daily living activities for patients, particularly in geriatrics, outpatient settings, and assisted living facilities.
- Collaborate with interdisciplinary teams to ensure coordinated care for patients.
- Maintain accurate medical records using appropriate medical terminology.
- Travel to various patient locations as required, providing care in diverse environments including acute care and inpatient settings.
Skills
- Proficient in infant care, with a strong understanding of pediatric nursing practices.
- Experience in travel nursing is preferred, showcasing adaptability to different healthcare environments.
- Knowledge of catheter care techniques and cardiac catheterization procedures.
