Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will work closely with patients, families, and healthcare providers to develop and implement individualized care plans that address the unique needs of each patient.
Responsibilities
- Conduct comprehensive patient assessments to evaluate health status and needs.
- Develop and implement personalized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed, including IV infusions and sterile processing.
- Perform phlebotomy for laboratory tests as needed.
- Assist with discharge planning to ensure a smooth transition from hospital to home care.
- Monitor patient progress and adjust care plans as necessary based on assessments.
- Provide education to patients and families regarding health management, medication administration, and self-care techniques.
- Document all patient interactions accurately in medical records in compliance with regulations.
- Collaborate with interdisciplinary teams to coordinate care for patients requiring memory care or ventilatory support.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Strong knowledge of physiology and medical office procedures.
- Experience with Minimum Data Set (MDS) assessments is preferred.
- Proficiency in patient assessment skills and discharge planning processes.
- Ability to perform sterile processing techniques effectively.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Previous experience in home health or similar settings is a plus.
- Must possess the ability to work independently while managing multiple priorities effectively.
