We are experiencing rapid growth and are currently seeking professional, compassionate Direct Care Workers to join our team. This essential role plays an intricate part in the success of our company. Providing exceptional care to our clients is the lifeblood of Care First Rehabilitation.
Setting/Facility
Pay: $16.00 - $17.00 per hour
Expected hours: 24 – 36 per week

Setting/Facility
- Group home setting; 6 in Troy and 1 in Farmington (must be able to work in both cities/ALL locations).
- If you’re working the day shift, you will be involved in our Day Program that the clients attend Monday-Friday from 9:00am-4:00pm. The clients have various therapies, catered lunch, activities, and outings each day during our program.
- $16/hour, to start, if not CNA certified.
- $17/hour, to start, if CNA certified or PCT certified.
- $18/ hour after 90 days of employment (must pass additional trainings and requirements).
- $1,000 quarterly attendance incentive program ($4,000/year).
- $150 stipend for successful employee referrals.
- Annual performance review with potential merit increase.
- Additional room for growth; A Supervisor position that you could be eligible for after 6 months of employment.
- Choose your 12 hour shift (7:00-7:30) AM and PM shifts.
- Great staff to client ratio.
- Ability to pick up open shifts for overtime (based on approval).
- Lunches and dinners are catered, by our chef, for the clients, Mon-Fri.
- Entire team of nurses, behavior therapists, and other therapists (SLP, OT, PT, CTRS, etc) who provide additional support to our clients and staff on a daily basis.
- You pick what holidays you work for time and half.
- Any hours over 40 hours per week is time and a half.

- Daily opportunities to take clients into the community when working the day shift.
- Providing full care to clients in a manner that considers safety, clients’ rights, comfort and a therapeutic environment.
- Documenting on an hourly basis via platforms including QuickMar and Internal Company Website as well as supplemental updates via When I Work platform.
- Assisting clients with daily activities (ADLs): light meal preparation, feeding, bathing/hygiene, toileting, etc.
- After 30 days of employment you will be trained on administering medications, administering peg tube feedings/flushes and glucose testing.
- Keeping the AFC homes clean and sanitized i.e. laundry, mopping, sweeping changing linen, taking out the garbage, cleaning and sanitizing bathrooms and kitchen areas, sanitizing equipment, organizing clients rooms and closets, dusting (deep cleaning).
- Supervising and engaging with clients during community outings such as recreational events and doctor appointments.
- Demonstrating compassion and patience for all clients on a daily basis.
- Have a valid driver's license.
- Must have reliable transportation.
- Must be able to work 12 hour shifts.
- Must be able to rotate weekends.
- Have a professional, calm, respectful demeanor/attitude.
- Be flexible - the house you’re scheduled at is subject to change, even if you picked up an open shift.
- Dress Code: Solid color scrubs, closed toe shoes, no crocs, no headwear.
- Must pass a background check within 30 days of hire (free of charge).
- Must have a negative TB test on file (free of charge).
- Must get a physical exam; Medical Clearance on file.
- No sleeping during either shift.
- Zero tolerance for drug/alcohol use.
- Be able to attend a one-day orientation (Monday) from 9:00am-4:00pm
- Must have good attendance; Only allowed 3 attendance violations per quarter (tardies, leave early requests, and unexcused absences).
- Demonstrate patience and compassion with our clients.
- Attend all zoom huddles during shift.
- Little to no phone usage that is not work related.
- Not able to leave for your break.
- Professional Caregiving: 1 year (Required)
- Previous experience working in a clinical setting (great to have but not required).
- Driver's License (Required)
- CNA certification (Not Required)
