Description
CFH is seeking an Intake Coordinator for our Shelter. The Shelter operates under contract with the New York City Department of Homeless Services (DHS) and, as a contract agency, has reporting responsibilities to DHS and is subject to inspections and audits by DHS, the New York State Office of Temporary and Disability Assistance and the Coalition for the Homeless of New York City.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Complete intakes for new clients and provide orientation to the program/shelter and DHS rules and regulations.
- Review logbooks at the start of each shift to ensure that all new client intakes are completed within 24 hours as required.
- Maintain required documentation in case records in written and computer databases (CARES) and update client's records in accordance with governmental and agency policy.
- Develop an individualized and comprehensive intake/assessment to fully identify for case management staff client needs.
- Collect client identification and other documents necessary during initial intake.
- Assist new clients in retrieving documents and/or property from former shelter providers as necessary.
- Collaborate with other shelter providers to request and obtain missing client information required (i.e. medical, psych, entitlement and/or housing information, etc...).
- Collaborate with internal medical staff to ensure new clients are registered with the clinic and initial evaluation appointments are issued.
- Directly collect client entitlement information and documentation and when unavailable request entitlement information (PA, SSI, etc.) via client escort.
- Deconstruct closed case files and discharged client files.
- Follow up with and submit referral packages to external shelter providers when clients are transferred.
- Input new clients on weekly data tracking spreadsheets.
- Complete minutes for staff meetings and develop and maintain meeting binders.
- Provide direct coverage for client reception area as needed.
- Participate in regular supervision, team meetings and staff training.
- Perform all/other duties as directed by management.
Qualifications
QUALIFICATIONS:
- HS/GED with three (3) years of experience with the homeless, mentally ill, substance abuse and/or low-income populations.
- Good writing, computer and communication skills required.
- Bilingual- Spanish is strongly preferred.
