Care For the Homeless (CFH) is seeking Licensed Practical Nurses for various locations within NYC.
SUMMARY: Under immediate supervision, supports providers (physicians, nurse practitioners, and physician assistants) in the provision of patient care; includes recording vital signs, discussing chief health concerns, performing testing, and administering injections; assists with procedures and documentation utilizing an EMR.
This employee will work as part of the PCMH team with the following essential duties and responsibilities:
- Patient care: records vital signs, discusses records chief health concerns, allergies obtains and documents patient historical health information in EMR; performs in-house lab tests; administers injections, stocks patient rooms with supplies.
- Service Collaboration: contacts pharmacies to order/re-order prescriptions; sets up referrals to specialty care and other community resources; collaborates with other service providers and colleagues to provide comprehensive services to patients.
- Administrative duties: answers phone; acts as patient advocate; attends regularly scheduled meetings; performs other general office duties as required.
- Quality Assurance: Participates in the CQI process.
- Education: supports patient education groups/sessions with provider and/or RN supervision; collaborates with the health education team to develop health curricula and patient education materials; conducts health fairs
- Other duties assigned within the Licensed Practical Nurse scope of practice and appropriate in-service training.
Qualifications
JOB QUALIFICATIONS:
Educational Requirements:
- Associate degree from an accredited Nursing Program or College
- A valid LPN license in NYS is required.
Experience:
- Minimum 2 years of experience in community health center work experience.
- Current CPR certificate is required.
- Working knowledge of EMR
- Knowledge of medical terminology
- Knowledge and understanding of HIPAA regulations and applicable confidentiality laws.
- Ability to maintain schedules.
- Ability to file and retrieve information.

- Excellent PC skills in MS Office: Word, Excel, PowerPoint & Outlook
- Excellent communication and interpersonal skills
- Ability to work independently with attention to detail
- Ability to respond to questions in a tactful and professional manner
