Description
Care For the Homeless (CFH) is a rapidly growing, entrepreneurial not-for-profit Federally Qualified Health Center and social service agency that serves the homeless and unstably housed people in New York City. CFH provides medical, behavioral, and oral health services through an ambulatory care network of 25 Article 28-licensed health centers, some of which are stand-alone and some of which are co-located at shelters, soup kitchens, and drop-in centers throughout NYC’s five boroughs. CFH also currently operates transitional residences for homeless persons and a safe haven that serves street homeless clients. CFH has additional residential programs in the development pipeline. Through its Policy and Advocacy Division, CFH maintains a leadership position in the public dialog about homelessness and advocates for policies to end homelessness.
We are looking to employ Physicians who practice primary care, psychiatry or pediatrics within the 4 boroughs. The right candidate will be an experienced leader and communicator who will respond to the needs of patients with compassion and empathy. The Physician will be responsible for assessing the health status of patients, diagnosing medical issues and providing long-term care for individuals. This is a desirable position for someone who has a strong background in medicine and a genuine interest in caring for others.
SUMMARY: Responsible for providing care and treatment to the center’s patients.
This employee will work as part of the PCMH team with the following essential duties and responsibilities:
Essential Duties and responsibilities:
- Participates as a member of committees, including peer review, as requested by the CMO
- Performs history and physical assessments as indicated by his/her specialty area
- Provides services in accordance with generally acceptable standards of medical practice
- Develops a plan of treatment/care for the patient that also includes education and follow-up visits
- Makes appropriate professional referrals, as indicated by the needs of the patient
- Communicates with other health care providers related to diagnosis, treatment and plan for the patient; professional communication is documented in the medical record
- Maintains current and complete documentation in the medical record that reflects the care and services provided to the patient
- Chart completion is consistent with CFH P/P
- Adheres to the center’s established policies and procedures
- Participates as a member of the medical staff and adheres to the medical staff bylaws
- Provides supervision and consultation to staff nurse practitioners/physician assistants as requested by the CMO
- Participates in the on-call and emergency coverage schedule, as directed by the CMO
- Participates in all managed care plans as requested by CFH
- Represents CFH positively in all professional and patient interactions
- Oversees care coordination with medical case management and health education as part of the PCMH team.
Loan Repayment Assistance: We are a member of the National Health Service Corps: NHSC.hrsa.gov. Licensed primary care clinicians in eligible disciplines can receive loan repayment assistance through the NHSC Loan Repayment Program (NHSC LRP). In exchange for loan repayment, you must serve at least two years of service at an NHSC-approved site in a Health Professional Shortage Area (HPSA).

- Meets all the requirements for appointment/reappointment as a member of the medical staff
- Deliver patient-centered care.
Salary : $180,000 - $210,000
